Where is my HSA Debit Card accepted?
Your HSA Debit Card is accepted at ATMs, general merchandise retailers (Kroger, Wal-Mart, Target, etc.) and medical service/product providers
such as doctor's offices, pharmacies, and medical supply stores. Please note that your HSA Debit Card cannot be used to make purchases at
certain retailers including clothing stores, restaurants and electronics stores.
Are there a maximum number of transactions per day?
There are no limits on the number of transactions per day. Those transactions cannot exceed the daily limits.
What limits apply?
ATM PIN-based transactions are limited to $500 a day. Signature-based transactions are limited to $1,000 a day.
Can I raise my limit?
Unfortunately, these limits cannot be changed. Should you need to make a purchase for a dollar amount in excess of $1,000 per day, we recommend using your HSA Checks.
How do I change or select a new Personal Identification Number (PIN)?
You should have selected your PIN at the time you completed your HSA Account-opening paperwork. If you do not remember your
HSA PIN, please contact the credit union at (513)352-3568 to select a new PIN.
When will I need to use my PIN?
You will need to use your PIN for ATM transactions. When making purchases, we always recommend that you purchase items using
the "credit" or signature-based mode. Signature-based purchases are always free. PIN-based purchases are $.25 per purchase.
For ATM transactions, you can use both PNC Bank ATMs and National City ATMs for FREE for a combined total of six (6) times per month; thereafter, you will pay $1.50 per transaction.*
How should I use my card?
Your card should only be used to pay for qualified medical expenses. If you use the card to pay for non-qualified medical expenses,
you are subject to income tax on those funds and IRS penalties. It is your responsibility to report all non-qualified expenses paid
using your HSA to the IRS when filing your annual income taxes.
Who do I notify if my card is lost or stolen?
If you need to report a lost or stolen HSA debit card, you can contact our Debit Card Department Monday through Friday, 7:30 a.m. - 5 p.m., EST at (513)352-3568 or (800)810-0221.

*You may use any combination of PNC Bank and National City Bank ATMs to perform your six (6) FREE ATM transactions per month (i.e., You cannot use PNC
Bank ATMs for free 6 times and National City ATMs for free 6 times.). If you use an ATM not owned by PNC Bank or National City Bank, you will be charged a
